Family Preservation Program
The Family Preservation Program (FPP) helps to strengthen families and keep children safely at home to prevent out of home placement. FPP is a collaboration between CHNK and the Kentucky Department for Community-Based Services (DCBS), so all referrals into the program are made by DCBS after confirming that a family meets all eligibility requirements.

Family Preservation may be right for families who may need ongoing, wraparound support to maintain the family unit.
What to expect
Families in the program work closely with their DCBS case worker and CHNK therapist to identify and work towards treatment goals. Therapy may include one or more of the following evidence-based practices in which CHNK treatment team members are trained: Trauma-Focused Cognitive Behavioral Therapy (TF-CBT), Motivational Interviewing (MI), Functional Family Therapy (FFT), and Multisystemic Therapy (MST). Case management services might be offered to help overcome additional barriers and link your family to needed resources.
Who is eligible?
Currently, all FPP referrals must come from Kentucky’s DCBS. Families eligible for services have an active/open case with the Cabinet for Health and Family Services (CHFS).
Who will be working with my family?
Our Family Preservation team is comprised of licensed professional counselors (LPCA, LPCC) and clinical social workers (CSW, LCSW). Our Targeted Case Managers are also part of the treatment team and partner with participating families.
Where does treatment take place?
Family Preservation typically takes place in the home or CHNK’s Outpatient Office.
How long is treatment?
Depending on your family’s individualized needs and progress in the program, you can expect to participate for 3 to 7 months.

Our Family Preservation Programs are an intense form of home-based therapy where proficient clinicians trained in specific Evidence Based Practices work in partnership with families to grow their skills and strengths. The goal of our FPP programs is to assist the families in building healthier connection and communication between it’s members in order to preserve family togetherness.
